Diabetes care at home is practical, personalised support for people living with diabetes who need help managing daily life safely and consistently in their own home. Most adults receiving this kind of support are living with type 1 or type 2 diabetes. Type 1 diabetes means the body cannot make insulin at all, while type 2 diabetes happens when insulin does not work properly or there is not enough of it, causing blood glucose levels to become too high. At Horizons Homecare, support is built around the individual. For some people, that means help with medication, blood sugar checks and meals. For others, it also means support with routines, personal care, getting to appointments, or delegated tasks where additional training is required. Home care does not replace medical treatment. It works alongside it, helping people stay safe, comfortable and supported at home while reducing pressure on family members.
Good diabetes management often depends on consistency. Medication, meals, hydration, activity, monitoring and general day-to-day habits all play a part in keeping blood glucose levels more stable and reducing the risk of avoidable problems. When routines slip, diabetes can become harder to manage. That is why home care can make such a difference. Reliable support helps people stay on top of the things that matter most, especially when fatigue, reduced mobility, memory issues or other health conditions make self-management harder. We can also work with doctors and pharmacies to help make sure medications are prescribed, collected and delivered where needed, lowering the risk of repeat medication issues and unnecessary stress. The aim is not to take independence away. It is to support the person in keeping as much independence as possible, while making daily diabetes management safer and more manageable.
Diabetes support does not only fit into one model of care. Some people need visiting care at set times during the day. Others may need more intensive support as needs change, especially if diabetes is being managed alongside other conditions. Horizons Homecare provides both visiting care and live-in care services. The routine is shaped around the individual, not forced into a fixed template. A person may begin with visiting care and later move to Live-in care at home if their needs become more intensive, or if that simply feels like the best fit for them and their family.
Home care can help with day-to-day diabetes management, but it does not replace clinical advice or emergency medical care. Low blood sugar, known as hypoglycaemia or a hypo, is usually a blood sugar below 4 mmol/L and needs to be treated quickly. High blood sugar, known as hyperglycaemia, can cause symptoms such as being very thirsty, needing to wee a lot, feeling more tired than usual, blurred vision, headaches and feeling sick. If someone becomes confused, drowsy, collapses, becomes unconscious, or you are worried they are seriously unwell, urgent medical help should be sought straight away. Severe hypoglycaemia can become an emergency, particularly if the person cannot safely eat or drink.
