What is diabetes care and support at home?

Diabetes care at home is practical, personalised support for people living with diabetes who need help managing daily life safely and consistently in their own home. Most adults receiving this kind of support are living with type 1 or type 2 diabetes. Type 1 diabetes means the body cannot make insulin at all, while type 2 diabetes happens when insulin does not work properly or there is not enough of it, causing blood glucose levels to become too high. At Horizons Homecare, support is built around the individual. For some people, that means help with medication, blood sugar checks and meals. For others, it also means support with routines, personal care, getting to appointments, or delegated tasks where additional training is required. Home care does not replace medical treatment. It works alongside it, helping people stay safe, comfortable and supported at home while reducing pressure on family members.

Why routine matters in diabetes care

Good diabetes management often depends on consistency. Medication, meals, hydration, activity, monitoring and general day-to-day habits all play a part in keeping blood glucose levels more stable and reducing the risk of avoidable problems. When routines slip, diabetes can become harder to manage. That is why home care can make such a difference. Reliable support helps people stay on top of the things that matter most, especially when fatigue, reduced mobility, memory issues or other health conditions make self-management harder. At Horizons Homecare, continuity matters. Familiar carers who know the person well are better placed to understand their routine, notice changes, and provide support in a calm, steady and reassuring way.

Support with medication, meals and blood sugar checks

Managing diabetes well often means keeping on top of several things at once. Medication needs to be taken correctly. Meals and snacks need to be more structured. Blood sugars may need to be monitored. Repeat prescriptions need to be ordered on time. When someone is tired, unwell or living with other conditions as well, all of that can become harder to manage consistently. Horizons Homecare can support with Medication assistance at home, meal preparation, routine building and blood sugar monitoring where this forms part of the agreed support plan. We can also work with doctors and pharmacies to help make sure medications are prescribed, collected and delivered where needed, lowering the risk of repeat medication issues and unnecessary stress. The aim is not to take independence away. It is to support the person in keeping as much independence as possible, while making daily diabetes management safer and more manageable.

Visiting care or live-in care

Diabetes support does not only fit into one model of care. Some people need visiting care at set times during the day. Others may need more intensive support as needs change, especially if diabetes is being managed alongside other conditions. Horizons Homecare provides both visiting care and live-in care services. The routine is shaped around the individual, not forced into a fixed template. A person may begin with visiting care and later move to Live-in care at home if their needs become more intensive, or if that simply feels like the best fit for them and their family.

Important to know

Home care can help with day-to-day diabetes management, but it does not replace clinical advice or emergency medical care. Low blood sugar, known as hypoglycaemia or a hypo, is usually a blood sugar below 4 mmol/L and needs to be treated quickly. High blood sugar, known as hyperglycaemia, can cause symptoms such as being very thirsty, needing to wee a lot, feeling more tired than usual, blurred vision, headaches and feeling sick. If someone becomes confused, drowsy, collapses, becomes unconscious, or you are worried they are seriously unwell, urgent medical help should be sought straight away. Severe hypoglycaemia can become an emergency, particularly if the person cannot safely eat or drink.

FAQs

Common questions

What does diabetes care at home include?+
It depends on the person. Support may include medication support, blood sugar monitoring, meal preparation, routine building, help with prescriptions, attending appointments, and day-to-day practical help that supports independence at home.
Can home care help someone stay independent with diabetes?+
Yes. For many people, the right support helps them stay independent for longer by keeping routines consistent, reducing missed medication, supporting safe meal preparation, and making day-to-day management feel less overwhelming.
Can carers help with blood sugar monitoring?+
Yes, where this forms part of the agreed care plan and the support is appropriate for the person's needs. If extra skills or delegated tasks are required, Horizons Homecare can source the right training and prepare the care team properly.
What are the signs of low blood sugar?+
Common signs of a hypo include feeling shaky, sweating, confusion, anxiety or irritability, blurred vision, hunger, tiredness and headaches. If someone with diabetes has symptoms of low blood sugar, they should check their blood sugar if they can and act quickly.
What are the signs of high blood sugar?+
Symptoms of high blood sugar can include going for a wee a lot, being very thirsty, feeling more tired than usual, headaches, blurred eyesight, feeling sick and sometimes unexplained weight loss.
Is diabetes care at home only for older people?+
No. Horizons Homecare supports adults aged 18+ in line with our registration. Diabetes can affect adults of different ages, and the support is tailored to the individual rather than based on age alone.
Can someone with diabetes have live-in care?+
Yes. Horizons Homecare provides both visiting care and Live-in care at home. Some people only need a few visits a week, while others need more continuous support. The routine is built around what works best for the client.
Will we see the same carers?+
Continuity is one of the things Horizons Homecare is known for. We aim to send the same carers where possible so the person receiving support can build trust and familiarity with their care team.
